Job Summary

  • The Medication Reconciliation Pharmacy Technician (MRPT) assists in accurately obtaining and verifying home medication lists for patients admitted to the hospital
  • The technician communicates with patients, families, pharmacies, and caregivers to ensure accurate medication histories are available to support safe and effective transitions of care

Essential Functions

  • Reviews and verifies home medication lists for all new admissions within 24 hours
  • Conducts patient and family interviews to confirm medication usage and dosage
  • Investigates medication histories using pharmacy records, prior admissions, and physician documentation
  • Assists with medication reconciliation at patient transfer or discharge when necessary
  • Ensures updates to the medication reconciliation module are accurate and completed timely
  • Prepares medication lists for physician review and ensures final documentation is available to patients and families upon discharge
  • Collaborates with pharmacists, nurses, and physicians to clarify discrepancies or incomplete information
  • Reports time spent on medication reconciliation tasks and supports quality data collection efforts
  • Places reconciliation documentation in the appropriate section of the patient chart as needed
  • Adheres to infection control practices, hospital dress code, and departmental policies
  • Maintains professional behavior, provides courteous patient interaction, and supports guest relations
  • Utilizes downtime productively and contributes to departmental teamwork

Knowledge/Skills/Abilities/Experience

  • Excellent interpersonal and communication skills with patients and care teams
  • Detail-oriented with strong documentation and data entry abilities
  • Understanding of drug names, therapeutic classes, dosage forms, and medication safety
  • Ability to work independently and maintain accuracy in a fast-paced clinical environment
  • Commitment to compassionate service, integrity, and teamwork
  • Frequent standing and walking
  • Occasional stooping and lifting of supplies or documentation
  • Requires visual and auditory acuity to assess medication records and communicate with patients
  • Hospital setting including Emergency Department and inpatient units
  • May be exposed to interruptions, emergency situations, and high patient volumes
  • PPE use required when entering patient care areas

Qualifications

Education

  • High school diploma or GED required
  • Completion of a medication reconciliation training and competency assessment required

Licenses/Certifications

  • Current registration as a Pharmacy Technician in the state of practice required
  • National certification through the Pharmacy Technician Certification Board (PTCB) required
  • Alternate credentials such as LPN or RN may be considered based on medication knowledge

Experience

  • Minimum of 3 years' experience as a Pharmacy Technician preferred
  • Strong working knowledge of medication names, dosages, forms, and administration routes required
  • Familiarity with medication reconciliation tools and hospital EMR systems preferred
