Description

Provides direct and indirect patient care activities under the direction of Senior Behavioral Health Unit Nursing Staff. Assists patients with activities of daily living, provides for personal care, comfort and assists in the maintenance of a safe and clean environment for an assigned group of patients.

Essential Functions:

·Performs patient care responsibilities that meet the psychosocial and physical needs specific to the standard of care for the geriatric patient and the patient's clinical condition.

·Assists in and accompanies patients in the admission, transfer and discharge process.

·Transports patients to various departments for ordered tests/procedures.

·Provides morning care, which may include bed bath or shower, oral hygiene, combing hair, back care, dressing patients, changing bed linen, cleaning bedside stand, straightening room and other general care as necessary throughout the day.

·Provides evening care which includes hands/face washing as needed, oral hygiene, back rubs, freshening linen, cleaning over bedside stand, straightening room and other general care as needed.

·Provides general nursing care such as positioning patients, lifting and turning patients, applying/utilizing special equipment, assisting in use of bedpan or commode and assisting patients with ambulation.

·Takes and records temperature, pulse, respiration, weight, blood pressure and intake-output.

·Communicates appropriately and clearly to team leader, coworkers, and physicians.

·Ensures patients are dressed properly and assists, as necessary. Ensures that used clothing is properly stored in bedside stand or on shelving.

·Ensures that all patients are clean and dry at all times.

·Assists patients with handling and care of clothing and other personal property (including dentures, glasses, contact lenses, hearing aids and prosthetic devices).

·Prepares patients for meals; serves and removes food trays and assists with meals or feeds patients, if necessary.

·Answers patients' call lights, anticipates patients' needs and makes rounds to assigned patients

·Makes rounds with outgoing shift; knows whereabouts of assigned patients.

·Makes rounds with oncoming shift to ensure the unit is left in good condition.

·Documentation meets current standards and policies.

·Provide daily socialization activities as scheduled, to specifically include daily Reality Orientation. Document and encourage patients to participate.

·Observes patients’ behavior and mental and physical conditions Reports any changes and unusual incidents observed to supervising Nursing Staff

·Knowledgeable of sterile techniques, infection control procedures.

·Knowledgeable of environmental risks for suicide and response process to follow in in the event of a suicide. • Participates in and contributes to Patients’ Treatment Plan and Patient Care Conferences

·Wears identification while on duty, uses computerized punch time system correctly.

Qualifications

Education:

  • High School Diploma or General Equivalency Diploma (GED), or equivalent, is required
  • Required Licenses: Certified Nursing Assistant

Required Certifications: 

  • American Heart Association Basic Life Support (BLS) Certification required within 90 days of hire or transfer.
  • Handle With Care Behavior Management System (HWC) Certification required within 90 days of hire or transfer.
  • Blood Glucose Monitoring Training Program Certification required within 90 days of hire or transfer.

Required Skills: 

  • Requirements will include successful completion of orientation checklist and skills/credentials/competency requirements of specific area of work.
  • Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
  • Able to communicate effectively and demonstrate service excellence through appropriate response and use of constructive problem-solving techniques when confronted with different situations; use of effective communication skills which foster positive outcomes.
  • Able to function independently and exercise judgment in interactions with physicians, interprofessional care team, and patients and their families. • Capable of being flexible and organized
  • Must be able to work in a stressful environment and take appropriate action.
