Case Management Social Worker

Watertown Regional Medical Center is now hiring for a PART-TIME Case Management Social Worker to support our

 hospitals and clinics.

The Case Management Social Worker will identify patients of WRMC appropriate for Case Management according to criteria determined by the Case Management Team.

Additionally you will:

• Monitor and coordinate continuum of care for patients
• Work collaboratively with the Case Management Team, WRMC staff, physicians, community members and other service providers to utilize and develop services that most appropriately meet patients' needs, while striving to sustain and/or improve the quality of life
• Perform utilization review, and participate in medical management program (pathways, protocols and guidelines) as appropriate

Qualifications:

• Bachelor’s Degree in Social Work, required. Master's preferred
• Licensed in the state of Wisconsin as a Certified Social Worker, required.
• Three years’ experience in healthcare
• Competent in discharge planning
