Job Summary

The Clinical Manager is responsible for overseeing the nursing and interdisciplinary teams in the subacute unit, including nursing staff, the Unit Social Worker, Activities Coordinators, and the Case Manager. This role ensures the delivery of high-quality patient care by administering the clinical program in compliance with regulatory requirements and internal standards. The Clinical Manager leads performance improvement efforts, manages staffing and training, and collaborates with leadership to maintain operational and fiscal accountability.

Essential Functions

  • Supervise and direct nursing care delivery 24/7 across the subacute unit.
  • Oversee assessments and individualized interdisciplinary care plans for each resident.
  • Conduct resident rounds and care conferences; ensure care plans are updated regularly.
  • Lead infection control practices and ensure adherence to universal precautions.
  • Evaluate quality of care, resident response to treatment, and nursing staff performance.
  • Ensure proper documentation of care and services in compliance with Title 22, OBRA, and HIPAA regulations.
  • Provide staff orientation and in-service training in coordination with the Director of Staff Development.
  • Participate in quality improvement initiatives and ensure compliance with facility and corporate standards.
  • Collaborate with Human Resources on hiring, performance reviews, and staff development.
  • Monitor staffing utilization and budget adherence; provide operational recommendations to leadership.
  • Communicate effectively with physicians, families, administrative teams, and external agencies.
  • Ensure adequate, qualified staff are scheduled to meet patient care needs.
  • Lead monthly unit meetings and performance improvement projects.
  • Coordinate audits, reporting, and outcome monitoring.

Knowledge/Skills/Abilities/Expectations

  • Strong leadership, clinical judgment, and team-building skills.
  • Knowledge of SNF/subacute regulations, care standards, and documentation requirements.
  • Excellent written, verbal, and interpersonal communication.
  • Ability to prioritize responsibilities, solve problems, and meet deadlines.
  • Demonstrated initiative and professionalism in all interactions.
  • Competency with electronic health records and standard office applications.

Pay Range: $119,000-$151,000/yr.

Qualifications

Education

  • Bachelor’s degree in Nursing preferred.

Licenses/Certifications

  • Current valid Registered Nurse license in applicable state.
  • Basic Life Support (BLS) certification required within time frame required by facility policy.

Experience

  • Minimum of three years of nursing experience required, with at least one year in a supervisory role.
  • Previous SNF or subacute experience required.
  • Experience caring for patients with technology dependency preferred.
