Job Summary

The Case Manager for the Value Based Program (VBP) functions as a core member of a collaborative care team, including the patient’s primary care provider and other clinic-based health providers. The role coordinates and supports healthcare transitions, manages quality and incentive-based programming, develops and oversees care plans, and fosters community and provider engagement. The Case Manager supports strategic and operational goals related to value-based care initiatives and chronic disease management.

Essential Functions

  • Coordinate care across the healthcare continuum to ensure smooth transitions for patients
  • Collaborate with discharge planning teams, physicians, and facilities for continuity of care
  • Engage primary care providers in transition and discharge planning processes
  • Conduct post-discharge outreach and support patients for at least 90 days
  • Educate patients on conditions, treatments, medications, and follow-up care
  • Document care plans and treatment recommendations in the patient record
  • Communicate and facilitate referrals to internal and external providers and services
  • Develop and manage care plans for chronic disease management
  • Serve as a patient experience advocate and support quality documentation practices
  • Support quality improvement programs and ensure provider engagement in quality initiatives
  • Act as clinic liaison for value-based care and community partnerships
  • Promote practice improvements through assessment, feedback, and planning
  • Participate in ongoing process improvement and support strategic initiatives

Knowledge/Skills/Abilities/Expectations

  • Strong communication and interpersonal skills
  • Ability to work collaboratively with multidisciplinary teams
  • Understanding of value-based care models and chronic disease management
  • Organizational and leadership skills in quality programming and patient-centered care
  • Adaptability and initiative in dynamic healthcare settings
  • Frequent standing and walking required. Manual dexterity needed for computer use and chart handling.
  • Occasional lifting and carrying of materials weighing 8–10 pounds.
  • Ability to stoop and bend for retrieving files.
  • Must be able to drive to area facilities.
  • Office and clinical care settings with adequate lighting and minimal hazards.
  • Regular patient and family interaction.
  • Participation in care coordination across various departments and external organizations. 

Qualifications

Education

  • High School Diploma or equivalent
  • Bachelor of Science in Nursing (BSN) Preferred

Licenses/Certifications

  • Certified or Registered Medical Assistant, or Licensed Practical Nurse (LPN), or Licensed Social Worker (BSW or MSW)
  • Must meet current licensure/certification requirements per role.

Experience

  • Minimum 3 years of clinical experience with demonstrated clinical judgment and communication skills
