Job Summary

The Licensed Practical Nurse (LPN) will provide a variety of patient care services, working as part of the clinical team under the supervision of a Registered Nurse (RN) or other qualified provider. Responsibilities include assisting with direct patient care, observing and documenting patient conditions, performing nursing procedures, and maintaining equipment and patient rooms. Under the direction of an RN, the LPN contributes to the assessment, planning, implementation, and evaluation of care for individual patients.

Essential Functions

Clinical Practice and Quality of Care

  • Assist in carrying out nursing interventions, collaborating with the care team, and incorporating appropriate standards of care to achieve optimal patient outcomes
  • Collect patient data and collaborate with the team to implement and evaluate the care plan
  • Support patient education by reinforcing instructions provided by the RN or provider, adapting to patient and family learning styles using the teach-back method
  • Perform nursing procedures, including specimen collection, wound care, medication administration, and assisting with urgent or emergent care
  • Document patient care activities, including observations and interventions, ensuring a comprehensive and integrated approach to care
  • Work with the team to monitor, adjust, and report on the patient’s response to care plans

Patient Care and Safety

  • Assist patients with activities of daily living (ADLs), including hygiene, nutrition, and ambulation
  • Monitor and report patient condition changes promptly to the RN or provider, including the effects of medications
  • Provide care to patients in diverse settings, including but not limited to swing beds, ambulatory care, Senior Behavioral Health and IV infusions
  • Ensure safe, ethical, patient-centered care that aligns with evidence-based practices and hospital policies
  • Provide care with the psychosocial needs of the patient and family in mind

Team Collaboration and Support

  • Contribute to department goals and quality improvement projects, including chart audits and patient satisfaction initiatives
  • Assist in the orientation and training of new staff and in updating LPN training materials
  • Float to other clinical settings and in other capacities such as a 1:1 patient companion as needed based on operational demands
  • Demonstrate willingness to collaborate with various departments and resolve problems effectively

Professionalism

  • Reflect the values of respectful, patient-centered care, and maintain a professional image at all times
  • Maintain competencies and participate in continuing education programs to support ongoing skill development
  • Actively participate in task forces or committees as requested, contributing to team projects and staff meetings

Equipment and Environmental Maintenance

  • Restock and maintain necessary supplies and equipment to ensure readiness for patient care
  • Address any malfunctioning equipment promptly and in accordance with hospital protocols

Knowledge/Skills/Abilities

  • Strong knowledge of general patient care and nursing procedures
  • Ability to observe, document, and report changes in patient behavior, symptoms, and vital signs
  • Proficient in providing direct care and support in clinical settings
  • Ability to work independently with supervision, while still collaborating effectively as part of a team
  • Effective communication skills with patients, families, and healthcare team members
  • Critical thinking skills and the ability to act quickly and appropriately in emergency situations
  • Ability to prioritize tasks, organize work efficiently, and meet deadlines

Qualifications

Education

  • Associates Degree in Practical Nursing – Required
  • One year of patient care experience in a healthcare facility – preferred

Licenses/Certifications

  • Valid State of LPN License
