Job Summary

  • The Nurse Navigator serves as a clinical guide and advocate for oncology patients throughout the continuum of care, from initial screening and imaging through diagnosis, treatment, and follow-up. This position provides professional nursing support to coordinate multidisciplinary care, assist in overcoming healthcare system barriers, and enhance the patient and family experience.

Essential Functions

  • Coordinates patient care across departments and providers from diagnosis through survivorship or end-of-life care
  • Consults with nursing staff and interdisciplinary teams to evaluate patient care, transitions, and discharge planning
  • Verifies that treatment plans align with patient needs and clinical standards
  • Provides education, emotional support, and advocacy for patients and families
  • Tracks and monitors patient progress, identifying and addressing barriers to care
  • Supports timely scheduling of diagnostic tests, treatments, and follow-up appointments
  • Participates in quality improvement initiatives related to patient navigation and satisfaction
  • Serves as a liaison between patients, providers, and community resources
  • Maintains detailed documentation and upholds patient confidentiality standards
  • Responds to questions and concerns from patients, families, and payers regarding care coordination

Knowledge/Skills/Abilities/Expectations

  • Excellent interpersonal and communication skills
  • Proficient in clinical documentation and electronic health records
  • Knowledge of oncology care standards, clinical pathways, and support services
  • Ability to prioritize tasks and manage multiple patient cases simultaneously
  • Strong organizational skills with attention to detail
  • Demonstrates trustworthiness, compassion, and professionalism
  • Collaborates effectively within interdisciplinary teams
  • Familiarity with performance improvement and patient satisfaction tools

Qualifications

Education

  • Licensed Practical Nurse (LPN) required
  • Bachelor of Science in Nursing (BSN) preferred

Licenses/Certifications

  • Current State or Compact Nursing License required
  • Basic Life Support (BLS) certification required upon hire and maintained thereafter

Experience

  • Strong critical thinking and decision-making skills
  • Ability to work independently and in high-stress clinical settings
  • Experience in oncology, case management, or patient navigation preferred
