Description

The Nurse Navigator is a clinical professional who is highly skilled and motivated. In this position, the ideal candidate will serve as a liaison between patients and clinical care providers on and off-site facilitating communication and coordination of care as needed while maintaining a professional image and exhibiting excellent patient care to our patients.

To perform this job, an individual must perform each essential function satisfactorily with or without reasonable accommodation.

1.Consults with nursing staff and multidisciplinary teams regularly to evaluate patient’s status and appropriateness of medical care, including admission, length of stay, transfer, and discharge.

2.Monitors patient and family satisfaction.Responds to questions and complaints from patients, family members, and payors regarding care.

3.Assures the accuracy and integrity of all data and collection processes.

4.Assist with data abstraction and submission for quality reporting, regulatory agencies and accreditation as required.

5.Analyzes data and processes to identify real or potential concerns. Provides ongoing strategicrecommendations related to training and coaching of providers and groups on program implementation, clinical compliance, and barrier resolution.

6.Assists in policy, metric, and process development and ensures that providers and operational teams have the most up to date documentation available.

7.Identifies patient/family education needs and ensures that patient/family members have adequate information to participate in transition planning.

8.Critically evaluates and analyzes physical and psychosocial assessment data.

9.Initiates and maintains communication and collaboration with physicians, social workers, care team leaders, staff nurses, other care giving disciplines, and patients/families to develop, implement, and evaluate a transition plan of care for each patient.

10.Conducts a comprehensive patient/family assessment and transition/home care planning evaluation upon program enrollment to initiate and maintain the patient's transitional plan of care.

11.Assists with quality reporting, regulatory and accreditation reviews and visits as assigned.

12.Participates in research surrounding transitional care. Identifies recurring clinical practice issues and contributes to the development of specific plans to address identified issues.

13.Demonstrates knowledge of research findings related to clinical specialty.

14.Participates in activities that support the advancement of care transitions, case management, and discharge planning through literature review, professional organizations, research, committee participations, etc. Consistently uses new knowledge, technology, and research in practice.

15.Screening and assessing patients by gathering and reviewing medical records.

16.Navigates high risk obstetric patients through the trajectory of care by assisting with facilitating patient appointments to providers.

17.Exhibits knowledge of current treatment details for the specific disease pertaining to the high-risk population of OB patients served while also demonstrating competency in, but not limited to care coordination, effective communication, and patient education.

18.Knowledge and understanding of the shared decision-making processes as it relates to patient evaluations and registry for insurance purposes.

19.Ordering and scheduling diagnostic testing, placing referrals, and scheduling appointments to coordinate care.

20.Maintains professional relationships with physicians, other care providers, co-workers, outside agencies, and hospital related staff and managers.

21.Willing to participate in community events such as health fairs, representing the organization in a positive professional manner.

22.Daily operations and communication to the team of physicians to develop, evaluate, and implement evidence based best practice that will drive quality outcomes.

23.Completes all annual training and educational requirements in a timely manner.

24.Performs other tasks, duties, and responsibilities as assigned or needed for the safety of our patients and the efficient operation of the office.

25.Assists other members of the department with their assignments and projects as required.


Qualifications

·Must possess a valid RN, BSN SC license.

·Excellent verbal and written communication skills.

·Self-starter, able to work independently as well with others.

·Able to build and maintain cross-functional relationships with others in and outside of the organization.

·Organized, and focused on accuracy and attention to detail. 

Education: RN required.

Experience: Requires critical thinking skills, decisive judgement, and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
