Job Summary

Under the general supervision of the Case Management Director, the supervisor of case management serves as a resource for the case management team and other departments. Responsible for providing direct supervision of the case managers.

Essential Functions

  • Conducts assessments and document conditions, observations, and actions.
  • Supports case management coordination of patient transfers to acute care facilities to ensure compliance with hospital policies/procedures and regulatory requirements.
  • Assist with managing the department in a cost-effective manner by utilizing current materials management techniques, maximizing human resources and continuously reevaluating the manner in which services are delivered.
  • Coordinates assignment of case management staff to meet required services in each area.
  • Supports case management coordination of patient transfers to ensure compliance with hospital policies/procedures and regulatory requirements.
  • Directs, coordinates, and provides case management to patients.
  • Facilitate patient and/or family counseling sessions; establish goals and care plans.
  • Participate as a contributing member, in the discharge planning process.
  • Serve as a liaison between hospital departments, to include discharge planning and social service.
  • Other duties as assigned.

Knowledge/Skills/Abilites/Expcetations

  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. 
  • Must be able to work in a stressful environment and take appropriate action.

Qualifications

Education

  • Associate Degree in Nursing (ADN) Required
  • Bachelor's degree Preferred

Licenses/Certifications

  • Professional Registered Nursing licensure in state of GA.
  • Basic Life Support (BLS)

Experience

  • Three (3) years of acute care experience
  • Previous Case Management experience
