Job Summary

• Provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice

Essential Functions

• Performs and/or coordinates patient care, including medication administration, specimen collection, clinical treatment, as week a other medical care treatment
• Documents patient care given
• Monitors, records and communicates patient condition as appropriate to care team, physician, patient and family
• Assists patients with understanding providers plan of care and necessary home instructions
• Collaborates as needed across disciplines to coordinate patient care including patient transfer, referral and spiritual/psychosocial support as needed
• Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition and circumstances
• Works as an advocate for the physical and emotional well-being of the patient
• Other duties as assigned

Education

• Graduate for an LPN program with a current State License

Licenses/Certifications

• Current LPN State license or Compact State License (if lives outside of SC)
• BLS – required upon hire and continuously thereafter

Experience

• Requires critical thinking skills, decisive judgement, and the ability to work with minimal supervision
• Must be able to work in a stressful environment and take appropriate action
