Job Summary:

Oversees daily operations for the Admission Coordinators. Manages schedules and evaluations for the Admission Coordinators. Serves as a liaison with marketing, business office and administration of the facilities. Coordinates and plans the daily operation for all pre-admission activities. Coordinates admission with other departments by collecting and distributing the appropriate information. Identifies information needed from the referring hospital to finalize the admission

Essential Functions: 

  • Coordinates the patient admission functions in keeping with the mission and vision of the hospital
  • Schedules all Admission Coordinators. Approves vacation requests and arranges for coverage. Completes performance evaluations of Admission Coordinators
  • Oversees daily activities of department
  • Coordinates twice daily admission and marketing calls
  • Intervenes in conflict issues
  • Cross-trains staff in all job responsibilities
  • Coordinates and facilitates the admission of patients from the referral sources. Identifies medical record information needed from referring facility. Assists in completing the Preadmission Clinical Evaluation (PACE) form to assist in evaluating appropriateness of admission. Applies appropriate clinical guidelines to pre-admission data. Notifies referring facility of admission decision. Expedites transfer. Facilitates and coordinates physician-to-physician communication as necessary. Identifies and communicates specific patient needs for equipment, supplies, pharmacy and consult services. Provides hospital team with needed information on pending/new admissions
  • Develops the necessary admission and patient day forecast for the institution in conjunction with the healthcare team. Develops appropriate analysis, using established reporting mechanisms and provides data and statistics as required, within the established time frames
  • Maintains a close working relationship with medical staff, interdisciplinary health team, and with external professional groups and agencies
  • Represents the company and hospital at professional seminars and community meetings
  • Explains the pre-admission and admission policy to families and referral sources as necessary
  • Maintains a state-of-the-art knowledge of areas of responsibility and develops and follows a program of continuing education
  • Conducts job responsibilities in accordance with the standards set out in the Company’s Code of Business conduct, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards. Maintains all information in a manner that ensures confidentiality and mees all regulatory requirements
  • Obtains verbal consent for transfer and admission of patient for receiving ScionHealth facility. Communicates with Clinical Liaison’s on a regular basis to ensure that all the information required for the entry into Meditech is complete. Contacts receiving ScionHealth facility when clinical and financial approval is complete. Notifies receiving ScionHealth facility of need for bed assignment
  • Insurance Patients: Designated AC will verify insurance coverage. Coordinate with insurance coverage on all coverage issues. Speak to the Medical Advisors of insurance company as needed and communicates approvals to facility with documentation regarding approval
  • MANG applications: Works with CLs to obtain all necessary information for MANG application and provides updates to facilities regarding status of MANG application

Knowledge/Skills/Abilities/Expectations:

  • Ability to discern, collect, organize, evaluate and communicate pertinent clinical information with effective verbal and written skills
  • Strong customer service skills
  • Computer application skills including work-processing, spreadsheets, presentations, email and calendar
  • Approximate percent of time required to travel: up to 10% as needed 
  • Must read, write and speak fluent English
  • Must have good and regular attendance
  • Performs other related duties as assigned

Qualifications

Education:

  • Bachelor’s degree in clinical area preferred

Licenses/Certifications:

  • Healthcare professional licensure required; that is, as Registered Nurse, LPN, Respiratory Therapist, Physical Therapist, Occupational Therapist or Social Worker 

Experience:

  • Three years’ healthcare experience
