The Licensed Practical Nurse / Licensed Vocational Nurse, LPN/LVN works closely with care staff to ensure timely response to calls for assistance. Oversees resident care including medication administration, obtaining resident vital sign and weight, and reporting any changes in condition to physicians, health care proxies and supervisors.
Weekend Only 7a-7p, 7p-7a
ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.
·Interviews resident to determine medical problem/condition and documents in chart for physician.
·Provides services in accordance with state assisted living standards and nurse practice act and maintains compliance to all personnel policies, established community policies and procedures, and Federal/State regulations and standards.
·Participates in pre-admission and move-in assessments, reviews and revise assessments with any change in resident’s condition and at frequency determined by the state
·Develop and revise service plans for residents based on needs assessments.
·Assist physician with resident examination, reviewing and transcribing physician orders and provide medical treatment as ordered.
·Administer medication to residents as ordered by physician.
·Supervises residents in self-care of medical equipment, such as oxygen, catheters, colostomies, etc.
·Maintains medications to include ordering, stocking and monitoring for expiration and documents appropriately for billing purposes.

Educational Requirements and Experience:
·License to practice nursing in the state of employment.
Education:
Graduate of an accredited program for Licensed Vocational/Practical Nurses.
Licenses/Certification:
Licensed Practical/Vocational Nurse License.
BCLS Certification.
Intravenous Blood Certification preferred.
Experience:
Minimum six months prior experience in an acute care setting preferred.
